1. There are 3 forms:
- Biographical Information (1 pg.)
- Travel Consent /Emergency Information (2 pgs.)
- Physical Packet (14 pgs.)
2. The Physical Packet must have doctor’s signature. If your student did a physical in May for Athletics, and you have a copy, please copy it and turn it in, keeping the other copy for your records.
3. Please fill out all information completely.
4. There are several spots for both parents and students to sign.
5. Please make copies of forms before your student turns them in.
6. Have your student turn them in to the clear plastic box in the band hall labeled “Band Forms”.
